Mielke, City ManagerA Date: December 19, 2000 Subject: Nomination of Council Member Fran Hesch to the Hennepin County Community Health Services Advisory Committee Attached you will find information about the Community Health Services Advisory Committee. This Committee reports to the Hennepin County Board of Commissioners on matters relating to the development, maintenance, funding, and evaluation of community health services. . There is currently an opening for a local official and I was contacted by Suzanne Gaines from Hennepin County Community Health Services with a request that we consider nominating Council member Hesch for a position on the advisory committee. This appointment is a two-year position and it is not necessary that an appointee be a member of the City Council, rather someone designated by the City to act as a local representative for local government. Hennepin County is anxious to have this position filled by the end of the year, and a nomination from Hopkins would help them make that goal. Assuming the City Council is willing to make the nomination, the City Manager will send a letter of nomination to the appropriate Hennepin county body. mayorccheschnominalion . . Community Health Services Advisory Committee Origin: The Committee was created by a 1979 County Board resolution (79-5-358) in accordance with the Minnesota Community Health Services Act (MN Statues 145A, Section 10, Subdivision 1 Ob). DutieslResponsibilities: In order to receive funding under the Community Health Services Act, a Community Health Services Advisory Committee is required to "advise, consult with, and make recommendations to the County Board (aka Community Health Board for purposes of MN CHS) on matters relating to the development, maintenance, funding, and evaluation of community health services". Composition: Three Local Officials or Municipal Representatives: ("Local Government Officials" means members of a board of county commissioners or a city council, or township board, or a school board, or other such officials who have responsibility for decision-making concerning health and related human services, or an individual from a municipality nominated by that municipality to serve as its representative) 1 Mpls; 1 Bloomington, Edina or Richfield; 1 Other suburban municipality. Five Health Providers: ("Provider" means any individual who is a licensed or credentialed health professional; an employee or representative of a licensed or certified health care institution or agency, health care insurer or health professional school; or a person with a material financial interest in the provision of health services) I Private physician nominated by Hennepin Medical Society; 1 Public physician nominated by H.C. Medical Center; I Registered . nurse nominated by Third District MN Nurses Assoc; 2 Other provider organization, e.g. hospital, professional association, managed care agency, community clinic, home care agency. Seven Consumer: ("Consumer" means a person who is not a licensed or credentialed health professional or the spouse of such person; a person who does not have a material financial interest in the provision of professional health services; and a person who is not directly related to health services planning and development, except as a J consumer member of health-related boards) 2 Mpls; 5 Suburban. Four Representatives from Other Community Organizations: ("Representative from Other Community Organizations" may include an employee or volunteer associated with groups such as these - voluntary health agencies, diversity culture organizations, advocacy groups, non- profit health organizations, schools, United Way, civic groups, social service agencies, neighborhood or community action groups) 1 Metropolitan Visiting Nurse Association; 3 solicited from groups such as - voluntary health agencies, diversity culture organizations, advocacy groups, non-profit health organizations, schools, United Way, civic groups, social service agencies, neighborhood or community action groups. One Hennepin County Commissioner: Chair of the County Board Health Committee. Term: 2 yrs starting Ian 1; see Other Information Meetings: The Committee shall meet at least three times a year and at the call of the chairman or a majority of the members. The committee generally meets once a month or once every two months for a total of about eight meetings a year. From time to time, subcommittees of the Advisory Committee are fonned to study specific concerns and report back to the full Advisory Committee.
