CHANGE ORDER
MAINSTREET RECONSTRUCTION
Proposed Action.
staff recommends adoption of the following motion: Move that
Council authorize Change Order 4-4. Project 90-04B. as it
concerns the Mainstreet Reconstruction Project.
Overview
Council at its March 17 meeting approved plans for relocation of
storm pipe onto Mainstreet between 11th and 13th Avenues and
ordered bids to be taken for the purchase of materials. staff at
that meeting recommended approaching Council with a change order
request that addressed the labor installation cost. Council at
its April 7 meeting approved a change order in the amount of
$110,177.50. Actual costs for street restoration exceeded the
original estimate and staff seeks authorization for a
supplemental change order to the storm sewer work.

0- What are the costs?
Change Order 4-4 reflects an 'additional sum of $12,900 for
completion of relocation of storm sewer onto Mainstreet
between 11th and 13th Avenues.
0 What is the justification for this change order?
Original plans for the pipe relocation called for bituminous
patching of the roadways in the areas of removals at 11th
and 13th Avenues. After trenching and pipe installation
were completed it was apparent that patching the disturbed
areas was not sufficient and complete street replacement was
necessary.

Dear Jim,
Pursuant to our conversation, I have documented herein Othe Change Orders submitted
with the July 1992paymertts for Lill;~ lvfaiastreet Improvement project.
On Hopkins Project 90-04B, we have submitted Change Order 4-4 for your approval.
This change order is for additional street removal and replacement in conjunction with the
60" storm sewer relocation. Our original plans demonstrated patching the areas of
necessary removals at 13th and 11th. However, during this process I met with Vern
Pankratz who agreed that complete street replacement was the logical.thing to do.
Consequently, additional removals and paving were necessary. Change Order 4-4 in the
lump sum amount of $12,900 accounts for this addition.
. On Hopkins Project 90-04C/D, we have submitted Change Order 6 for your approval.
This change order covers several items. First, street lights on 7th and 8th Avenues were
hooked in to the Mainstreet lighting system, although this was not part of the. original
plans. This was done to accommodate the elimination of the old feed points on these
streets. This work was accomplished at the request ofVern and Willie. Second, Killmer
Electric was requested by Public Works and Community Developmentto add some
electrical outlets in the planter beds at the plaza. This work was accomplished per this
request. These items are all covered by Change Order 6, which is requesting lump sum
payment of$1468.94.
