CR 99-188 Authorize Change Order & Final Payment Random Sidewalk Repair CITY OF ~ September 29, 1999 NOPKINS Council Report 99-188 Authorize Change Order and Final Payment Random Sidewalk Repair Proposed Action. Staff recommends adoption ofthe following motion: Move that Council authorize a change order and adopt Resolution 99-99, Resolution Accepting Work and Ordering Final Payment, Random Sidewalk Repair Project 99-04. Overview. The contractor has completed work associated with repair of City sidewalks. The contractor has performed on punch list items to the City's satisfaction. Primary Issues to Consider. Change order issues Net additional work required on sidewalk repair amounted to $3,492.27. Repair of walks on Mainstreet accounted for the increased costs. Brick paver repair necessitated removal and replacement of more sidewalk panels than anticipated. Concrete removal also called for more sawcutting which was bid relatively high at $6.00 per linear foot. Funding for this additional cost can be covered by savings made on the 1999 seal coat proj ect. The total project cost is $27,191.37. Accepting work and final payment The contractor has met his contract work obligations and the work is done satisfactorily. Documents still needed are final lien waivers, IC-134 withholding forms and a signed Certificate of Final Completion. The contractor is currently gathering that documentation. Staff recommends approval of final payment with a check tendered upon receipt of the above forms. Resolution 99-99 Adoption of this resolution formally acknowledges completion of work, orders final payment and initiates a specified warranty period on the contract work. Supporting Information. Date CITY OF HOPKINS HENNEPIN COUNTY, MINNESOTA RESOLUTION NO. 99-99 RESOLUTION ACCEPTING WORK & ORDERING FINAL PAYMENT- RANDOM SIDEWALK REPAIR PROJECT 99-04 WHEREAS, pursuant to a written contract signed with the City on July 7, 1999, Gunderson Brothers Cement Construction, Inc. has satisfactorily completed the improvement of random sidewalk repair, commonly known as City Project 99-04, in accordance with such contract. NOW, THEREFORE, be it resolved by the City Council of the City of Hopkins, Minnesota: a. 1. The work completed under said contract is hereby accepted and approved. 2. Order for final payment on such contract is hereby ordered, taking the contractor's receipt in full. 3. The specified warranty period on contract work shall commence upon the adoption of this resolution. Adopted by the City Council ofthe City of Hopkins, Minnesota, this 5th day of October, 1999.
