Staff recommends the following motion: Move to approve Resolut RZ93 -20 y ��rpendtng approval of a conditional use permit to allow an adult day care center at 34 10th Avenue South. Overview. (;pie adult Day Care provides adult day care for individuals in the western suburbs. Open Circle did operate in Hopkins several years ago in the old Harley Hopkins School. Now tX e s located in Edina and is proposing to move to 34 10th Avenue South. This biting was previously occupied by the Knights of Columbus. The Zoning Ordinance requires the following condition: o said operation shall be l The current operation is licensed by the Minnesota Department of Human Services and will continue to be licensed. o What are the specifics of their operation? With the new facilities the program is proposing to have a license capacity of 60 persons per day. The hours of operation are from approximately 9:00 a.m. to 3:00 p.m. Participants in the program will receive two snacks and a noon meal during the day. The new facility would also provide bathing facilities. Please see the attached narrative prepared by Open Circle which explains their program further. o What is the proposed access to the building? Access to the building will have to be changed to allow for disabled persons to enter. The applicant is proposing to remove part of the curbing and slope the sidewalk up to the entrance. The Public Works department has reviewed the proposed access and will not allow the curbing removed and the sidewalk sloped to the buildings entrance. The applicant will have to modify the access to be acceptable to the staff. Staff is continuing to work with the applicant on access to the building. In order not to delay the applicant another month, staff is recommending that a condition is added to the conditional use permit which requires the applicant provide a plan detailing access to the building that meets building code requirements and is acceptable to staff. Alternatives. 1. Recommend approval of the conditional use permit to run an adult day care center at 34 10th Avenue South. By recommending approval the City Council will consider a recommendation of approval. 2. Recommend denial of the conditional use permit to run an adult day care center at 34 m 10th Avenue South. By recomending denial the City Council will consider a recommendation of denial. Continue for further information. T p 6 5 CONDITIONAL USE PERMIT APPLICATION Narrative for Open Circle Adult Day Program Open Circle is an adult day care program licensed by the Minnesota department of human services to provide a therapeutic daily activity program for frail elderly and disable adults living in the western suburbs of the Twin City area. Our plan for this space is to increase licensed capacity to 60 persons per day. The average age of participants is 80 and they generally live both alone or with families in Hopkins, Minnetonka, or St. Louis Park. The program provides transportation which picks the participants up in the morning to arrive at the center around 9:00 AM and then they are taken home at 3:00 PM. During the day two snacks and a noon meal are served. The program provides the following services to participants who attend: a. Health monitoring b. Nutritional counseling and hot meals c. Medication administration and monitoring d. Personal care services such as foot care and minicures e. Assessment case management f. Respite and support for caregivers g. Nursing services and evaluation h. Therapeutic recreation and music therapy programming i. Transportation to and from the program j. Social services such as referral and counseling k. Treatment by Audiologists, Speech, Occupational Physical Therapists 1. Individual goal setting and treatment planning with quarterly care conferences m. Caregiver support through educational sessions and social activities n. In the new facility, we would provide bathing Our inclusive cost is based on a sliding fee scale up to $40 per day and includes transportation. We maintain a staffing ratio of 1:5 participants, excluding the Director. In addition we provide the services of special consultants such as a Music Therapist, Foot Care Specialist, Craft Specialist, and Community Education Teachers. We also have various volunteers who assist us with programming. All Open Circle staff have Bachelor's or Master's degrees and certification in CPR, First Aid and Medication Administration. Open Circle maintains contracts with the Minnesota Alzheimer's Association, the Hennipen County Community Health Pre- Admission Screening Department, and the Hennipen County Community Services to Seniors. CITY OF HOPKINS Ill/ Hennepin County, Minnesota RESOLUTION NO: RZ93 -20 RESOLUTION MAKING FINDINGS OF FACT AND. RECOMMENDING APPROVAL OF AN APPLICATION FOR A CONDITIONAL USE PERMIT FOR AN ADULT DAY CARE CENTER AT 34 TENTH AVENUE SOUTH WHEREAS, an application for a conditional use permit CUP93 -9, made by West Suburban Adult Day Care Center at 34 Tenth Avenue South, is recommended for approval. WHEREAS, the procedural history of the application is as follows: 1. That an application for a conditional use permit CUP93 -9 was filed with the City of Hopkins on September 20, 1993. 2. That the Hopkins Planning Commission, pursuant to published and mailed notices, held a public hearing on the application and reviewed such application on October 26, 1993: all persons present at the hearing were given an opportunity to be heard. 3. That the written comments and analysis of the City staff and the Planning Commission were considered. NOW THEREFORE, BE IT RESOLVED that the application for conditional use permit CUP93 -9 is hereby recommended for approval based on the following Findings of Fact: 1. That the applicant meets the requirements for a day care facility as outlined in the Zoning Ordinance. BE IT FURTHER RESOLVED, that application for CUP93 -9 is hereby recommended for approval based on the following conditions: 1.
